One thing I would put extra emphasis on as well is education. I think many people within a company are interested in engaging or contributing, but simply don't know where to start. I hold lunch and learns which are a great way to get people up to speed.

#4 is tough! Love to get more coverage of the options and possibilities. I've found about 25 different providers for everything from the networks to the forums and not one comparison from anyone with experience.

I've been working with Buddypress at http://boruma.com
Love it, but it is only in its first release so SEO needs work and I'd like to see more integration between blogs, groups, and the forum (they use BBPress) to tie everything together. Free is a big plus.

This is a great top level post. I might add a 6th item. That’s create an executable strategy. It goes much along the lines with determining your objectives, but it creates a strategy for the project at hand and then how you will position yourself for further objectives. Great Job.
